New Applicant Check List

Please provide the following documentation when applying for membership.

_____ GCRS Application (available at WWW.greenerescue.org)

_____ 3 Letters of Recommendation

(exclude immediate family, roommates or references listed on GCRS application; preferably a work supervisor

_____ Copy of Drivers License

_____ Copy of Certifications (EVOC, EMT, etc.)

_____ Driving Record

Checklist for New Member Orientation

Each of the following topics are to be discussed with new member prior to first duty shift.

_____ Uniform (GCRS shirt, EMS pants/dark blue, shoes/boots, Sweatshirt)

_____ Shift Assignment (starts promptly at 1800; duty coverage etc).

Give member a copy of duty roster & phone list.

_____ Walkie Numbers (will be assigned by 2^nd LT)

_____ Identification Badge (picture taken-sent to UVA)

_____ Truck Orientation (complete truck inventory form with member)

_____ Building Orientation

(kitchen, living room, sleeping quarters, classroom, wash room,

medical supply room)

_____ By-Laws & SOP’s (found at www.greenerescue.org)

_____ Copy of Probationary Manual; review probationary period;

precepting; release as AIC or driver

_____ Meetings: Training; Business; Monthly BLS or ALS, CE Hours
